Grant funding was sent directly to you to complete the purchase of your approved seizure detection device or app subscription. Now that you have completed your purchase, you are required to upload your receipt or proof of purchase using the form below. This documentation is necessary for the Foundation’s records and confirms that grant funds were used appropriately and in accordance with your signed agreement. Please complete the short form below and upload your receipt(s).
 
If you chose not to move forward with the purchase, you are required to return the check so the funds can be reallocated to another family in need. Please contact [email protected] to discuss the return of your grant.
